Surveys will be available to be completed by parents, community members, faculty, staff and students in grades 3-12. Please view the links below for survey content and format. Also attached is a passive parent permission form to be returned by March 27th if parents do not want their student to participate in the survey. Students will complete the survey in school during the week of March 30th.
