The Juniata Valley School District has announced Kindergarten registration for children who will be five years of age before September 1, 2019 and reside within the Juniata Valley School District. Registration has been scheduled for Monday, April 22 and Tuesday, April 23, 2019 at the Juniata Valley Elementary School.
Parents of eligible children who have not received a registration packet must call the elementary school at 669-4422, ext. 1501 to receive a packet and appointment time.
Immunization requirements for admission to school include the following:
•4 doses of tetanus, diphtheria and acellular pertussis (1 dose given after the fourth birthday)
•4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose given)
•2 does of measles, mumps, rubella
•3 doses of hepatitis B
•2 doses of varicella (chickenpox) or evidence of immunity
Along with the immunization records, copies of the following are required:
•child’s birth certificate
•social security number
•proof of residency
